Special Info
All blood bank samples and requisitions must be labeled with the patient's full name and a unique permanent ID number, social security number, or patient's date of birth. If red cell units are ordered, the sample must be labeled with the date and initials of the phlebotomist drawing the sample. ABO Group, Rh Type, Antibody Screen and antibody identification must be known prior to ordering red cells. Those specimens with a positive antibody screen will have antibody identification performed and will incur additional fees.
